Abstract

Cardiomegaly is an enlargement of the heart that can be assessed using the Cardiothoracic Ratio (CTR) on posteroanterior (PA) chest radiographs. Although CTR remains widely used in radiological practice, information regarding the distribution of CTR values based on digital radiographic archives in local hospital populations remains limited; therefore, local clinical data are needed to describe cardiac size characteristics within specific healthcare settings. This study aimed to describe the profile of cardiomegaly assessment using CTR based on archived chest radiographs at the Radiology Department of RSUD Panembahan Senopati Bantul. A retrospective descriptive study with a quantitative approach was conducted using PA chest radiograph archives from January to April 2026 that met the predefined inclusion and exclusion criteria. CTR measurements were performed using the measurement tools available in the Picture Archiving and Communication System (PACS) and analyzed descriptively according to CTR categories and patient characteristics. This study contributes local PACS-based data describing the distribution of cardiac size among the hospital population, which may support initial radiological evaluation and improve understanding of cardiomegaly patterns in routine clinical practice. Among 414 analyzed chest radiographs, most samples showed CTR values within the normal category (50.72%), while 16.18% were categorized as cardiomegaly. The overall mean CTR was 44.64 ± 5.51%, and cardiomegaly was more frequently observed among female patients and older age groups. These findings indicate that CTR measurement on PA chest radiographs provides preliminary information regarding cardiac size distribution; however, its interpretation should be correlated with clinical conditions and other supporting examinations.

ABSTRAK

Kardiomegali merupakan pembesaran jantung yang dapat dinilai secara sederhana melalui pengukuran Cardiothoracic Ratio (CTR) pada radiografi thorax proyeksi posteroanterior (PA). Meskipun CTR masih banyak digunakan dalam praktik radiologi, informasi mengenai distribusi nilai CTR berbasis arsip radiografi digital pada populasi rumah sakit lokal masih terbatas, sehingga diperlukan data berbasis pelayanan klinis nyata untuk menggambarkan karakteristik ukuran jantung pada suatu populasi. Penelitian ini bertujuan untuk mengetahui profil penilaian kardiomegali menggunakan CTR pada arsip radiografi thorax di Instalasi Radiologi RSUD Panembahan Senopati Bantul. Penelitian ini menggunakan desain deskriptif retrospektif dengan pendekatan kuantitatif terhadap arsip radiografi thorax PA periode Januari - April 2026 yang memenuhi kriteria penelitian. Pengukuran CTR dilakukan menggunakan measurement tools pada sistem Picture Archiving and Communication System (PACS), kemudian dianalisis secara deskriptif berdasarkan kategori CTR dan karakteristik pasien. Penelitian ini berkontribusi dalam menyediakan gambaran lokal berbasis data PACS mengenai distribusi ukuran jantung pada populasi rumah sakit, yang dapat mendukung evaluasi radiologi awal dan pemahaman pola temuan kardiomegali dalam praktik klinis. Hasil penelitian terhadap 414 arsip radiografi menunjukkan bahwa sebagian besar sampel memiliki nilai CTR dalam kategori normal (50,72%), dengan 16,18% termasuk kategori kardiomegali. Nilai rata-rata CTR seluruh sampel adalah 44,64 ± 5,51%, sedangkan kasus kardiomegali lebih banyak ditemukan pada pasien perempuan dan kelompok usia lanjut. Temuan ini menunjukkan bahwa pengukuran CTR pada radiografi thorax PA dapat memberikan informasi awal mengenai distribusi ukuran jantung, namun interpretasinya tetap memerlukan korelasi dengan kondisi klinis dan pemeriksaan penunjang lainnya.
