Abstract

ABSTRACT

Vesicocele, or urinary bladder diverticulum, is a structural abnormality of the bladder wall characterized by protrusion of the mucosa and submucosa through a defect in the detrusor muscle layer. BNO-IVP examination generally uses AP supine, oblique (RPO/LPO), and post-void AP erect or PA prone projections. However, at the Radiology Department of RSUD Banyumas, modifications were identified, including the addition of lateral and oblique projections at 60 minutes and the use of AP supine projection during the post-void phase. This study aimed to describe the BNO-IVP procedure in patients with vesicocele, explain the role of the additional projections, and determine the rationale for using AP supine projection during the post-void phase. This qualitative study employed a case study approach and was conducted at the Radiology Department of RSUD Banyumas from October 2025 to May 2026. Data were obtained through direct observation, documentation, and in-depth interviews with one radiographer and one radiologist, followed by data reduction, presentation, and conclusion drawing. The BNO-IVP examination included a plain abdominal radiograph, intravenous contrast administration using bolus injection, post-contrast images at 5, 15, 30, and 60 minutes using AP supine projection, followed by lateral and oblique projections at 60 minutes, and a post-void AP supine image. The main findings showed that the lateral and oblique projections helped overcome anatomical superimposition and improved visualization of diverticular morphology, while AP supine post-void imaging provided adequate information regarding bladder emptying and residual urine within the diverticulum. These modifications supported the visualization of vesicocele and diagnostic assessment.

ABSTRAK

Vesicocele atau divertikel vesika urinaria merupakan kelainan struktural dinding kandung kemih yang ditandai dengan penonjolan mukosa dan submukosa melalui defek lapisan otot detrusor. Kondisi ini diperiksa menggunakan BNO-IVP dengan proyeksi standar meliputi AP supine, oblique  (RPO/LPO), serta post void proyeksi AP erect atau PA prone. Namun, di Instalasi Radiologi RSUD Banyumas ditemukan penambahan proyeksi lateral pada menit ke-60 dan penggunaan proyeksi AP supine pada fase post void, dua variasi yang seharusnya tidak ada dalam protokol standar dan belum terdokumentasi dalam penelitian terdahulu. Tujuan penelitian ini bertujuan untuk mendeskripsikan prosedur pemeriksaan BNO-IVP pada pasien Vesicocele di Instalasi Radiologi RSUD Banyumas, mengetahui peranan penambahan proyeksi lateral, serta mengetahui alasan penggunaan proyeksi AP supine pada fase post void. Penelitian ini merupakan kualitatif dengan pendekatan studi kasus yang dilaksanakan di Instalasi Radiologi RSUD Banyumas dari Oktober 2025 hingga Mei 2026. Data dikumpulkan melalui observasi langsung, dokumentasi, dan wawancara mendalam dengan satu radiografer dan satu dokter spesialis radiologi. Analisis data meliputi tahap pengumpulan data, reduksi, penyajian, dan penarikan kesimpulan. Prosedur BNO-IVP dilakukan dengan foto polos abdomen, injeksi media kontras intravena metode bolus injection, foto post-kontras menit ke-5, 15, 30, dan 60 proyeksi AP supine, dilanjutkan proyeksi lateral dan oblique  pada menit ke-60, serta foto post void proyeksi AP supine. Penambahan proyeksi lateral dan oblique  terbukti mengatasi superposisi anatomi dan memungkinkan visualisasi morfologi divertikel dari berbagai sudut pandang. Penggunaan AP supine pada fase post void memberikan informasi diagnostik yang memadai mengenai kemampuan pengosongan vesika urinaria dan sisa urin dalam kantong divertikel. Modifikasi teknik BNO-IVP yang diterapkan di RSUD Banyumas terbukti efektif dalam mengidentifikasi Vesicocele dan mendukung penegakan diagnosis yang akurat untuk menentukan derajat keparahan serta kebutuhan intervensi lebih lanjut.
