Abstract

Lipid profile examination is used to help assess the risk of cardiovascular disease. LDL level examination can be performed using the indirect method (Friedewald Formula) and the direct method (homogeneous test). The indirect method is more economical and is still frequently used in laboratories, but its accuracy is starting to be questioned at high triglyceride levels. This study aims to determine the accuracy of LDL levels using the indirect method (Friedewald Formula) compared to the direct method (homogeneous test) at triglyceride levels of 200-300 mg/dL. This study used a comparative analytical quantitative design with a paired cross-sectional approach. The study sample used primary data, namely the serum of patients with triglyceride levels of 200-300 mg/dL examined using the indirect and direct methods on the same sample. Data analysis was performed using the Wilcoxon test, mean %Bias, Bland-Altman, and linear regression. The results showed that there was no significant difference between the indirect and direct methods with a significance value (p > 0.05). The average %Bias value of -0.3962% still meets the acceptance limits of the National Cholesterol Education Program (NCEP). Bland-Altman analysis showed that most of the data were within the limits of agreement. Based on the results of the study, the indirect method (Friedewald Formula) has good accuracy and relatively comparable results with the direct method (homogeneous testing) at triglyceride levels of 200-300 mg/dL.

ABSTRAK

Pemeriksaan profil lipid digunakan untuk membantu menilai risiko penyakit kardiovaskular. Pemeriksaan kadar LDL dapat dilakukan menggunakan metode indirect (Friedewald Formula) dan metode direct (homogeneous assay). Metode indirect lebih ekonomis dan masih sering digunakan di laboratorium, tetapi akurasinya mulai diragukan pada kadar trigliserida tinggi. Penelitian ini bertujuan untuk mengetahui akurasi kadar LDL metode indirect (Friedewald Formula) terhadap metode direct (homogeneous assay) pada kadar trigliserida 200–300 mg/dL. Penelitian ini menggunakan desain kuantitatif analitik komparatif dengan pendekatan cross-sectional berpasangan. Sampel penelitian menggunakan data primer, yaitu serum pasien dengan kadar trigliserida 200–300 mg/dL yang diperiksa menggunakan metode indirect dan metode direct pada sampel yang sama. Analisis data dilakukan menggunakan uji Wilcoxon, mean %Bias, dan Bland-Altman. Hasil penelitian menunjukkan tidak terdapat perbedaan bermakna antara metode indirect dan metode direct dengan nilai signifikansi (p > 0,05). Nilai mean %Bias sebesar -0,3962% masih memenuhi batas keberterimaan National Cholesterol Education Program (NCEP). Analisis Bland-Altman menunjukkan sebagian besar data berada dalam batas kesepakatan. Berdasarkan hasil penelitian, metode indirect (Friedewald Formula) memiliki akurasi yang baik dan hasil yang relatif sebanding dengan metode direct (homogeneous assay) pada kadar trigliserida 200-300 mg/dL.
