Abstract

Mental health issues in adolescents are an important public health concern because they can impair academic performance, social relationships, and long-term well-being. Schools occupy a strategic position in early detection as they can reach adolescents broadly, yet data on emotional and behavioral problems and their association with symptoms of depression, anxiety, and stress at the school level remain limited. This study aims to analyze the relationship between emotional and behavioral problems and symptoms of depression, anxiety, and stress among students at SMP Negeri 4 Ubud as baseline data for developing school-based mental health initiatives. The study utilized a cross-sectional analytical observational design with a total sampling technique for students participating in the screening under the SENYUM Program in 2026. Emotional and behavioral problems were assessed using the Strengths and Difficulties Questionnaire (SDQ), while symptoms of depression, anxiety, and stress were evaluated using the Depression Anxiety Stress Scale for Youth (DASS-Y). A total of 292 students aged 12–15 years were analyzed. Overall, 24.3% of students had total SDQ difficulties in the borderline or abnormal category, while 22.9% exhibited mild or more severe DASS-Y symptoms in at least one subscale. Total SDQ difficulties were strongly correlated with total DASS-Y (ρ=0.79; p<0.001). Emotional symptoms represented the SDQ domain with the strongest and most consistent relationship with depression, anxiety, and stress after multivariable adjustments. These findings demonstrate a significant link between emotional and behavioral difficulties and psychological distress symptoms among students. The SDQ and DASS-Y can be utilized complementarily in school-based mental health screenings, provided that screening outcomes are accompanied by appropriate follow-up and clinical evaluations as needed.
