EFEKTIVITAS COGNITIVE AND BEHAVIORAL INTERVENTIONS DALAM MENGURANGI RASA TAKUT JATUH (FEAR OF FALLING) PADA PASIEN LANJUT USIA: SEBUAH LAPORAN KASUS BERBASIS BUKTI

Elderly individuals have a high risk of falling along with declining physical condition. This decline in physical condition can result in the emergence of a fear of falling which then has an impact on various psychological problems. This can reduce the quality of life of individuals, so further intervention is needed to reduce psychological problems related to falling. Therefore, this evidence-based case report was compiled to determine the effectiveness of cognitive and behavioral therapy interventions in elderly individuals who have a fear of falling. Methods: Article searches were conducted in five scientific journal databases, namely PubMed, Wiley Online Library, Google Scholar, EbscoHost, and Cochrane. Article searches were conducted with specific keywords to obtain appropriate results. Study selection was carried out using predetermined inclusion and exclusion criteria. Results: From the selection process, 1 systematic review study was obtained that was in accordance with the clinical question. The study showed moderate but significant results in reducing fear after short-term intervention (SMD: ?0.30, 95% CI ?0.50, ?0.10) and in the long term (SMD: ?0.29, 95% CI (?0.49, ?0.09). Conclusion: Cognitive and behavioral interventions can be an intervention option to overcome fear of falling in the elderly.
ABSTRAK
Individu lanjut usia memiliki resiko jatuh yang tinggi seiring dengan menurunnya kondisi fisik. Penurunan kondisi fisik ini dapat mengakibatkan munculnya rasa takut jatuh yang kemudian berdampak pada berbagai masalah psikologis. Hal tersebut dapat menurunkan kualitas hidup pada individu, sehingga sangat diperlukan intervensi lebih lanjut untuk mengurangi masalah psikologis terkait dengan jatuh. Oleh karena itu, laporan kasus berbasis bukti ini disusun untuk mengetahui efektivitas intervensi terapi kognitif dan perilaku pada individu lanjut usia yang memiliki rasa takut jatuh (fear of falling). Metode: Pencarian artikel dilakukan di lima database jurnal ilmiah yaitu PubMed, Wiley Online Library, Google Scholar, EbscoHost, dan Cochrane. Pencarian artikel dilakukan dengan kata kunci spesifik untuk mendapatkan hasil yang sesuai. Seleksi studi dilakukan dengan menggunakan kriteria inklusi dan eksklusi yang sudah ditentukan. Hasil: Dari proses seleksi didapatkan 1 studi tinjauan sistematis yang sesuai pertanyaan klinis. Studi menunjukan hasil pengaruh sedang namun signifikan di dapatkan pada penurunan rasa takut setelah dilakukan intervensi jangka pendek (SMD: ?0.30, 95% CI ?0.50, ?0.10) dan pada jangka panjang (SMD: ?0.29, 95% CI (?0.49, ?0.09). Kesimpulan: Intervensi kognitif dan perilaku dapat menjadi pilihan intervensi untuk mengatasi rasa takut jatuh pada lanjut usia.
